PDF/A - Make DocumentReference fixtures consistent w.r.t. description and title

Description

Some DocumentReference fixtures contain a .description, while others (for instance medmij-pdfa-DocumentReference-kwalificatie1) don't. The .description element should be added to each fixture (especially since the title of the document maps to this element according to IHE MHD).

Moreover the content of .content.attachment.title is not consistent: DocumentReference 2 through 5 have a title of the form 'Example PDF - {actual title}', while DocumentReference 1 doesn't. This should be made consistent as well (e.g. by either adding 'Example PDF -' to the title of DocumentReference 1, or removing it from the title of DocumentReference 2 through 5).
